Since, due to the lack of a license for medical activities and the lack of paramedical workers in most preschool educational institutions, there are certain difficulties in organizing work in such a way as not to violate the law and the requirements of supervisory authorities.



Definition of immunization

  • Immunoprophylaxis of infectious diseases - a system of measures taken to prevent, limit the spread and eliminate infectious diseases through preventive vaccinations. (Article 1 of the Federal Law of September 17, 1998 N 157-FZ "On Immunoprophylaxis of Infectious Diseases"). -



Why is vaccination needed?

  • Vaccination is an important part of the fight against many infections.

  • Infections against which there are vaccines are called controllable because the level of their disease depends on the number of vaccinated among the entire population.

Immunity

  • Active artificial, specific immunity, is the result of vaccination, persists for a long time, after infections, sometimes for life (measles, mumps), it is:

  • - specific - as it develops against a specific vaccinated infection.

  • Through vaccination, herd immunity is created, which is a barrier to the spread of infection and the emergence of an epidemic.



Criteria for the effectiveness of vaccination

  • 1. The level of immunological activity of vaccines

  • (the ability to induce an immune response in humans), is determined by the number of individuals who have developed protective antibody titers from the number of all vaccinated as a percentage, as well as by the concentration of antibodies in the blood. Should be 93-100%

  • 2. Vaccination coverage of the population - ensures epidemiological effectiveness, should be at least 95%

  • 3. The epidemiological effectiveness of vaccination is assessed by the ratio of the incidence rate of an infection in the group vaccinated against it to the incidence rate among unvaccinated individuals.

  • Preventive vaccination plans in children's educational organizations are:

  • medical workers (doctor, nurse of the organization) together with the nurse-cartographer of the medical and preventive organization.

Guidelines MU 3.3.1891-04 "Organization of the work of the vaccination room DP, CIP, vaccination teams"

  • The general management of the organization and work of vaccination teams is assigned by the head of the medical facility to the deputy chief physician of the polyclinic or the head of the polyclinic department, who exercises daily control over the work of vaccination teams.

Functional duties of a nurse

  • 1. Vaccination is carried out by a nurse, who is on the staff of the preschool educational institution in close cooperation with the doctor of the medical facility attached to the educational institution on a territorial basis according to the schedule.

  • 2.Nurse DOW:

  • informs the parents of children about the required vaccination and the need for it, possible reactions of the body to the administration of drugs;

  • obtains written consent or refusal of prophylactic vaccinations

  • informs parents about the date and time of vaccination



  • conducts preventive vaccinations in strict accordance with the instructions for the use of drugs, in compliance with all the rules of asepsis and antisepsis

  • records all information about the vaccination (type of drug, dose, series, control number) in the medical records:

  • - journal of preventive vaccinations (f64u),

  • - medical record of a child attending a preschool educational institution, a general educational institution

  • (f. 026-u)



  • provides the correct storage of a vaccine during the working day in the conditions of cold at a temperature of 2 - 8 °C.

  • monitors the timely replenishment of anti-shock agents and their expiration dates;

  • after the preventive vaccination, the child is monitored for the period specified in the instructions for use of the corresponding vaccine preparation (approximately 30 minutes)



  • notes in medical documents the nature and timing of general and local reactions, if any

  • provides disinfection and destruction of vaccine residues, used ampoules, syringes

  • generates a report on the vaccinations carried out and provides it to health care facilities on a territorial basis



Functional duties of a pediatrician

  • Receives a vaccine, syringes and all necessary materials at the medical facility;

  • after vaccination, establishes observation of the child for a period specified in the instructions for use of the corresponding vaccine preparation (approximately 30 minutes)



  • provides medical assistance in the event of a complication to the introduction of the vaccine,

  • registers a post-vaccination complication, if necessary, hospitalizes the patient or sends him for a consultation to a medical facility;



  • Action algorithm in the absence of a license to carry out medical activities in an educational institution



  • Vaccination is carried out by the vaccination team of the health facility that provides medical care on a territorial basis in accordance with the schedule agreed with the head of the preschool educational institution.

  • The general management of the organization and work of vaccination teams is assigned to the management of the medical facility, which exercises daily control over the work of vaccination teams.



The composition of the vaccination team

  • The vaccination team includes a doctor and a nurse

  • The vaccination team works in full force, with full equipment, prepared for vaccinations and emergency care.



Functions of OU during vaccination

  • When conducting vaccinations by vaccination teams in preschool educational and general educational institutions, the head of the preschool educational institution provides:

  • Providing lists of attending or studying in these organizations and written consent or refusal of preventive vaccinations;

  • Preliminary notification of parents (legal representatives) of minors to be vaccinated about the place and time of vaccination through medical workers (hygienic education instructor).



  • 3. Organizes the vaccination site, providing conditions for asepsis in accordance with sanitary and hygienic requirements.



Functional duties of the doctor of the vaccination team

  • checks the equipment and readiness for work of the vaccination team;

  • conducts a survey, examination of the child before prophylactic vaccination

  • draws up admission or medical exemption from vaccination with fixing information in the medical record of a child attending a preschool educational institution, a general educational institution (f. 026-y),

  • carries out medical supervision of the vaccinated within 30 minutes;

  • provides medical assistance in the event of a complication to the introduction of the vaccine,



  • registers a post-vaccination complication in accounting forms, if necessary, hospitalizes the patient or sends him for a consultation to a medical facility;

  • in the event of an unusual reaction or complication to the introduction of the vaccine, immediately informs the head of the educational institution, health care facility and sends an emergency notice (f. 58) to the territorial center of Rospotrebnadzor within the established time limits;

  • supervises the work of a nurse, ensuring the correctness of vaccination and maintaining documents.



Functional duties of a nurse (vaccinator):

  • - receives a vaccine, syringes and all materials for one day of work of the brigade;

  • ensures the correct storage of the vaccine during the working day under cold conditions at a temperature of 2 - 8 ° C.

  • receives and monitors the timely replenishment of anti-shock agents and their expiration dates;

  • vaccinates in accordance with the instructions for use of the vaccine;

  • participates in the provision of medical care in case of post-vaccination complications;



  • ensures disinfection and destruction of vaccine residues, used ampoules, syringes;

  • return vaccine residues and other materials to the clinic on the same day.
